Form C13 should be completed if you are applying for the Warren & Evelyn Hotz Memorial Scholarships - (12) $3,000 Requirements: You must be a graduating senior of the Omaha Public Schools and will pursue a post secondary education. These scholarships are restricted to a person who show a clear intent to work to defray a substantial portion of his/her education. The student shall have demonstrated good citizenship in high school an shall have participated in school activities to the extent allowed by his/her work schedule. The student must rank in the top one-third of the class. One of your letters of recommendation must be from an employer.
